The Civil Rights Act of 1964 prohibits discrimination in employment because of race, color, creed, religion, sex or national origin. Federal law also prohibits other types of discrimination such as age, citizenship, disability, veteran status, attainment of benefits, and participation in union activities. The laws of most state and many localities also prohibit some or all of the above types of discrimination as well as some additional types including, but not limited to, discrimination based upon ancestry, marital status, parental status, sexual orientation, or source of income. The Fair Credit Reporting Act imposes restrictions with respect to credit data. The position for which you are applying the requested information is needed for a legally permissible reason, including, without limitation, national security requirements, affirmative action, bona fide occupational qualification or business necessity.
It is my understanding that the Recreation Commission will investigate my work history and may verify all data given in my application for employment, related papers or oral interviews. I authorize such investigation and the giving and receiving of any information requested by the Recreation Commission and I release from liability any person giving or receiving any such information. I understand that falsification of data so given or other derogatory information discovered as a result of this investigation may prevent my being hired, or if hired, may subject me to immediate dismissal.
The Recreation Commission makes every effort to accommodate individual preferences; the Commission may at times make the following conditions mandatory, a rotating work schedule, a work schedule other than Monday through Friday or a varied work schedule dependent upon seasonal needs.
I understand this is an application for employment and that no compensation agreement is being offered. I understand that if I am employed, such employment is at will and that the Recreation Commission can change wages, hours, benefits and conditions at any time. I have read and understand the above.
